The user. An adult with extensive experience using computers. Currently staring at a dialog box.
Background
A modal window has appeared. It contains information the user did not request and is now required to dismiss before continuing. The modal has a "Close" button, centered at the bottom, labeled clearly.
The user is looking in the upper-right corner for an x.
What Was Found
The x is not in the upper-right corner. This does not mean the x does not exist. It means the x is not in the upper-right corner, which is where all x-buttons have historically lived. The upper-right corner is empty. The user is experiencing this as a problem.
The governing rule: when a digital control creates doubt, additional clicking is recognized as a valid audit method.
The x is the expected close mechanism. Its absence in the upper-right corner is being treated as a design failure rather than a design choice.

Findings
The close button at the bottom says "Close." It has not been tried until Phase 7. The x was never present.
The user was correct that the modal could be closed. The user was incorrect about which control performed the closing. The time between arrival of the modal and its dismissal was forty seconds, thirty-seven of which were dedicated to searching for a button that does not exist.
The interface has technically already answered the question. The user was not asking the interface. The user was checking whether the interface had placed the answer in its historically expected location.
Status
Eventually success is trusted only when the interface has been manually doubted in the approved order.
The modal is closed. The findings stand.
