On the Practice

Someone writes down when a symptom started. The note is a precaution. Medical conversations ask for timelines, and memory is unreliable when uncertainty is high, so writing it down is reasonable and the kind of thing a careful person does once.

By the third entry, the notes have a structure.

Stage One

The first note reads: "headache, after lunch." This is sufficient. It answers the question it was written to answer. It is a sensible document.

Stage Two

A second entry is added. The second entry includes a time. The first entry did not include a time. The first entry is revised to include a time. Every medical conversation asks for the same sequence, so the sequence gets written down until the sequence is the notes.

Stage Three

A second person copies the behavior because the repetition looks like experience. The second person's notes are also structured. The structure is not identical to the first person's structure. A brief conversation occurs about format. The more detailed format prevails.

Consequence

The group eventually treats the copied behavior as the safest available standard. The notes are checked before any call to a medical professional. The notes are also checked after any call, to confirm that what was reported matches what was written.

The Field Condition

An inconvenient ritual becomes official when stopping it would require a new decision. Nobody stopped taking symptom timeline notes because stopping them would mean deciding that the notes were no longer necessary, and nobody wanted to be the person who made that decision on the day the notes turned out to be necessary.

The Contradiction

The notes seek certainty in a fast-changing situation. Certainty expires. A note written yesterday is a record of yesterday. It is also the best available record, so it remains in use.

Current Status

Notes continue. The format has been revised four times. It is described by the household as the system.
